Job summary

CenterWell Home Health is seeking a dedicated Physical Therapy Assistant to provide in-home care under a supervising Physical Therapist. The role focuses on administering therapy plans, tracking progress, and ensuring safe delivery of services in patients’ homes.

Ideal candidates will have an active PT assistant license, CPR certification, and strong teamwork abilities, with excellent English communication. A valid driver’s license and reliable transportation are required.

Qualifications

Current state license to practice Physical Therapy.
Valid driver's license, auto insurance and reliable transportation.
Current CPR certification.
Experience collaborating with a team of healthcare professionals.
Comprehensive knowledge of geriatric population changes.
Read, write, and speak fluent English.

Responsibilities

Administer the physical therapy treatment plan to patients, working onsite in their homes as developed by the Physical Therapist.
Administer non-complex active and passive manual therapies and modalities.
Document client records and submit clinical documentation to the office weekly.
Attend educational meetings and coordinate department in-services.
Notify Physical Therapist or Branch Director of equipment failures, supply shortages, or staff or client changes.
