Job summary

Qualifications

• Physical Therapist license issued by the state of Wisconsin.
• Basic Life Support (BLS) certification within 6 months unless exempt.
• 3 years of physical therapy rehabilitation experience.

Responsibilities

• Conduct comprehensive physical therapy evaluations and establish care plans.
• Manage assigned caseload and communicate effectively with physicians.
• Participate in chart audits and facilitate discharge planning.
