ProHealth Care in Wisconsin is seeking a Physical Therapist to coordinate an interdisciplinary plan of care in a home health setting. You will collaborate with patients, families, nurses, social workers, physicians and community agencies to assess plans and ensure timely, quality, cost-effective care.

Flexible hours and 24-hour accountability may be required to meet patient and family needs. Candidates must have WI licensure, a PT degree and CPR/BLS certification, with home health experience

Qualifications

• Bachelor's Degree in Physical Therapy or higher.
• State of WI Physical Therapist licensure.
• Valid State of WI Driver's License with acceptable driving record.
• Current BLS (CPR) certification or ability to obtain upon hire.
• At least 6 months experience in a home health setting preferred.
• At least 1 year physical therapy experience preferred.
