Job summary

At Home Healthcare, part of LHC Group and a member of the Optum family, seeks a Physical Therapist Assistant to support patients in home care through exercise instruction and education. You will follow a plan of care, coordinate with the supervising Physical Therapist, and impact patient well-being.

Responsibilities include guiding patients in exercises, assessing progress, and collaborating with nurses, doctors, and family members to optimize outcomes.

Qualifications

• Current, licensed, registered, or certified to assist in the practice of physical therapy in state of practice.
• Current driver's license and vehicle insurance or access to dependable transportation.
• Available to work a weekend rotation.

Responsibilities

• Follow state laws governing home care PT; perform only permitted procedures; report changes to supervising PT.
• Instruct patients in active and passive exercise, gait, ADLs, and mobility training.
• Observe and record patient responses; coordinate care with healthcare team and supervisor.
• Educate patient, family, and caregivers on therapy within PTA scope.
• Function in diverse home settings and with clients of all backgrounds.
