Job summary

Capitol Home Health is a family-owned home health agency serving Central Texas, with offices in Austin, Temple and San Antonio. The Physical Therapist will evaluate referred patients in a home care setting, formulate treatment plans, and deliver therapies defined in the plans while coordinating with physicians and case managers.

The PT is expected to have clinical experience in home health, hold an active PT license in the state, and be prepared to travel locally.

Qualifications

• Graduate of a Baccalaureate, Master's or Doctorate program in Physical Therapy program accredited by a Physical Therapy accreditation.
• Current and unencumbered licensed to practice as a Physical Therapist within the state of employment.
• Two (2) years of appropriate experience as a Physical Therapist. Community/home health experience is preferred.
• Demonstrates good verbal and written communication and organization.
• Possess and maintain valid CPR Must have obtained in-person with a practical skills evaluation as part of the final test.
• Access to reliable transportation.

Responsibilities

• Evaluate and assess needs of referred patients in a home care environment.
• Formulate and implement treatment plans in collaboration with physicians and case managers.
• Provide therapy services defined in treatment plans and use creativity within allowed scope.
• Work cooperatively with physicians, case managers, and patients to support recovery.
