Qualifications

• Current Physical Therapy licensure in state of practice.
• Current CPR certification.
• Current driver’s license with access to a vehicle or reliable transportation.
• Ability to work with diverse patient populations in home health settings.

Responsibilities

• Evaluates patient functional status and therapy needs with physician assistance for care plans.
• Provides PT services within state practice guidelines and plan of care.
• Monitors patient response to treatment and reports changes to the supervising nurse or physician.
• Delivers treatment to relieve pain and restore function while maintaining standards of practice.
• Educates patients and families about plan of care, limitations, and progress.
• Completes visits with progress notes and submits per policy.
• Develops PT plan of care within state guidelines.
• Reports outcomes and frequency of care projections.
