Job summary

Mississippi HomeCare of Kosciusko, part of LHC Group and within the Optum family, seeks a Physical Therapist in Home Health. You will assess and evaluate patients, determine a treatment plan, implement interventions to improve well-being, and monitor progress.

Responsibilities include evaluating functional status, assisting in plan of care, and educating patients and families. PT licensure, CPR, a valid driver’s license, and reliable transportation are required.

Qualifications

• Current Physical Therapy licensure in state of practice.
• Current CPR certification.
• Current driver's license and vehicle insurance, access to a dependable vehicle, or public transportation.
• Ability to function in any home situation regardless of age, race, creed, color, sex, disability, or financial condition of the client.

Responsibilities

• Evaluates the patient's functional status and physical therapy needs; assists physician in developing the plan of care.
• Provides services within the scope of practice and plan of care.
• Observes and reports patient response to treatment to supervising nurse or physician.
• Treats patients to relieve pain, develop and restore function, and maintain maximum functioning.
• Instructs and informs patient and family/caregivers regarding plan of care, functional limitations, and progress toward goals.
• Visits patients according to Plan of Treatment; submits progress notes per policy.
• Completes all patient evaluations and develops PT plan of care within state guidelines.
• Reports outcomes of evaluation, goals, and projected frequency of care.
