Job summary

ExcelaCare Home Health in Hobbs, NM is seeking a Physical Therapy Assistant (PTA) to provide skilled rehabilitation services on a PRN per visit basis under the supervision of a licensed Physical Therapist. The PTA will perform planned treatments, monitor patient progress, document care, and participate in team meetings.

Travel to patients' homes is required, with flexibility in scheduling and adherence to agency policies.

Qualifications

• Education: PTA graduate from an accredited program.
• Experience: Two years as a PTA within the last five years.
• Licensure: Current PTA license; CPR certification; valid driver’s license and auto liability insurance.

Responsibilities

• Performs services delegated and supervised by the Physical Therapist to meet patient goals.
• Assists patients with exercises and activities and uses assistive devices as prescribed.
• Follows Plan of Care and agency policies under PT supervision.
• Meets with the PT regularly to update progress and needs.
• Participates in case conferences and team meetings.
• Makes visits as scheduled and participates in on-call rotation.
• Maintains patient rights and confidentiality.
• Documents timely and accurately according to policy.
