Job summary

Signature Albany Home Health in Albany, OR is seeking a dedicated Home Health Physical Therapist to evaluate patients in their homes and create individualized treatment plans. You will deliver skilled physical therapy, collaborate with physicians and nurses, and educate patients on home exercise programs.

Current Oregon PT license and a valid driver's license are required; home health experience is preferred but not mandatory.

• Current Oregon Physical Therapist License.
• Valid driver's license and reliable transportation.
• Home health experience preferred, but not required.
• Strong communication and clinical skills.

Responsibilities

• Evaluate patients and create individualized treatment plans.
