Job summary

HiredFirst is seeking a Physical Therapist for a Home Health contract in Oregon. The role requires evaluating patients and creating individualized home treatment plans to restore and improve physical, social, and mental function.

Responsibilities include communicating care and discharge plans with patients and families, supervising therapy assistants as needed, and maintaining accurate documentation while ensuring safety and quality standards.

Qualifications

• DPT or equivalent is required.
• Active Oregon PT license required.
• BLS certification required.
• Valid driver’s license and 21+ years old.

Schedule:

Day Shift: Monday – Friday, 8:00 AM – 5:00 PM

Position Summary & Responsibilities:

Evaluate patients and develop individualized home treatment plans

Requirements:

Doctor of Physical Therapy (DPT) or equivalent

Active Physical Therapist license in Oregon

BLS certification (AHA or equivalent)

Valid driver’s license and at least 21 years of age
