Job summary

The role requires a Doctor of Physical Therapy, active licensure, and strong collaboration with a multidisciplinary team. Travel to patients’ residences is required in the Charlotte, NC area.

Qualifications

• DPT required and valid PT license in the applicable state.
• Maintains continuing education per state standards.
• BLS for Healthcare Providers recommended.

Responsibilities

• Manage daily patient caseload with timely care and documentation.
• Collect and analyze home health assessments and EMR data.
• Identify high-risk patients and coordinate case conferences.
• Develop and implement plan of care with physician collaboration.
• Supervise PT aides and home health aides as applicable.
• Communicate updates and changes to the multidisciplinary team.
• Ensure treatments comply with state practice acts and guidelines.
