Job summary

UnitedHealth Group's MS HomeCare division seeks a Physical Therapist to perform assessments, develop and implement therapy plans, and guide patients toward improved mobility and function in home health settings. You will collaborate with physicians and nurses to ensure quality care and compliant documentation.

Responsibilities include evaluating patients, planning care, providing therapies, educating families, and reporting progress.

Qualifications

• Current Physical Therapy licensure in state of practice.
• Current driver’s license and vehicle insurance or access to a dependable vehicle or public transportation.
• Preferred: CPR certification or ability to complete within 90 days of hire.

Responsibilities

• Evaluates patient’s functional status and PT needs.
• Helps develop therapy plan of care with physician.
• Provides PT services within scope of practice and plan of care.
• Observes and reports patient response to treatment.
• Treats patients to relieve pain and restore function.
• Educates patient and family on plan of care and progress.
• Completes visits, progress notes, and plan of care within guidelines.
• Reports outcomes and frequency of care.
• Function in diverse home settings.
