Job summary

UnitedHealth Group leads a Home Health team seeking a Physical Therapist to assess patient needs related to mobility and function in a home setting. You will help determine treatment plans, perform interventions, and evaluate progress, coordinating with nurses and physicians.

You will evaluate functional status, develop care plans, and ensure care adheres to state guidelines, with pay structures including per-visit and hourly components for productivity.

Qualifications

• Current Physical Therapy licensure in state of practice.
• Current driver's license and vehicle insurance, access to a dependable vehicle, or public transportation.

Responsibilities

• Evaluates the patient's functional status and physical therapy needs.
• Assists the physician in the development of the therapy plan of care
• Provides services within the scope of practice as defined by state laws and plan of care
• Observes and reports patient response to treatment to the supervising nurse/physician
• Treats patients to relieve pain, develop and restore function, and maintain maximum functioning
• Instructs patients, families, and care teams about the plan of care and progress
• Visits patients according to Plan of Treatment and submits progress notes
• Completes all patient evaluations and PT plan of care per state guidelines
• Reports outcomes of evaluation, goals, and anticipated care frequency
