Job summary

UnitedHealth Group is seeking a Physical Therapist in Home Health to assess and evaluate patient needs related to mobility, endurance, pain, and other conditions. You will help shape the treatment plan, perform interventions to improve well-being, and monitor progress.

Responsibilities include evaluating functional status, delivering care within state practice acts, coordinating with the health care team, and documenting outcomes.

Qualifications

• Current Physical Therapy licensure in state of practice.
• Current driver's license and vehicle insurance, access to a dependable vehicle, or public transportation.
• Preferred CPR certification or ability to complete within 90 days of hire.

Responsibilities

• Evaluates the patient's functional status and physical therapy needs.
• Provides services within the scope of practice as defined by state laws and plan of care.
• Observes and reports patient response to treatment and changes in condition.
• Treats patients to relieve pain, develop and restore function, and maintain maximum functioning.
• Informs patient and family about plan of care and progress toward goals.
• Visits patients per the Plan of Treatment and submits progress notes per policy.
• Completes all patient evaluations and develops the PT plan of care within state guidelines.
• Reports outcomes of evaluation, goals, and frequency of care.
