Job summary

UnitedHealth Group seeks a Physical Therapist for Home Health to perform patient assessments, determine treatment plans, and implement interventions to improve mobility and well-being. You will evaluate progress and coordinate with physicians, nurses and families to ensure quality care.

Qualifications

• Current Physical Therapy licensure in state of practice.
• Current driver’s license and vehicle insurance, access to a dependable vehicle, or public transportation.
• CPR certification or ability to complete within 90 days of hire.

Responsibilities

• Evaluates the patient's functional status and physical therapy needs. Assists the physician in the development of the therapy plan of care.
• Provides services within the scope of practice as defined by state laws and in coordination with the health care team.
• Observes and reports the patient’s response to treatment and changes in condition to the supervising nurse or physician.
• Treats patients to relieve pain, develop and restore function, and maintain maximum functioning.
• Instructs patient and family/caregivers about plan of care and progress toward goals.
• Visits patients per Plan of Treatment and submits progress notes per policy.
• Completes all patient evaluations and develops the PT plan of care within state guidelines.
• Reports outcomes of evaluation and frequency of care.
