Job summary

Louisiana HomeCare of River Parishes, part of the LHC Group and Optum family, seeks a Physical Therapist to assess patients’ functional status in home health and develop treatment plans. You’ll perform interventions to improve well-being and track progress with the health care team.

The role emphasizes collaboration with physicians and nurses, adherence to state PT guidelines, and providing education to patients and families to support goals and continuity of care.

Qualifications

• Current Physical Therapy licensure in the state of practice.
• Current driver’s license and access to reliable transportation.
• CPR certification or ability to complete within 90 days of hire (preferred).

Responsibilities

• Evaluate patient functional status and PT needs; assist in developing the plan of care.
• Provide PT services within scope of practice and plan of care.
• Observe and report patient response to treatment to nurse/physician.
• Treat to relieve pain, restore function, and maintain maximum functioning.
• Educate patient and family on plan of care, limitations, and progress.
• Visit patients per Plan of Treatment; document progress notes per policy.
• Complete all evaluations and develop PT plan within state guidelines.
• Report outcomes and anticipated frequency of care; ensure accessibility in all settings.
