The role offers a combination of per-visit point pay and hourly pay within a full-time schedule, with total cash compensation ranging from $86,607 to $129,910 annually for full productivity.

Qualifications

• Current Physical Therapy licensure in state of practice.
• Current driver's license and vehicle insurance, access to a dependable vehicle, or public transportation.
• Current CPR certification or ability to complete within 90 days of hire.

Responsibilities

• Evaluates the patient's functional status and physical therapy needs; assists physician in plan of care.
• Provides services within state scope of practice and plan of care.
• Observes and reports patient response to treatment to supervising staff.
• Treats patients to relieve pain, restore function, and maintain maximum functioning.
• Instructs patient and family about plan of care and progress toward goals.
• Visits patients per Plan of Treatment and submits progress notes per policy.
• Completes evaluations and develops PT plan of care within state guidelines.
• Reports outcomes of evaluation, goals, and frequency of care.
• Functions in diverse home settings without discrimination.
