Job summary

UnitedHealth Group, via Atrinity Home Health, seeks a Physical Therapist for Home Health to perform assessments and evaluations of patient care needs related to functional status, mobility, endurance, pain and other conditions. Based on assessment, you will help determine a treatment plan and intervene to improve and maintain well-being.

You will work with the health care team, document visits, and ensure care plans meet state guidelines.

Qualifications

• Current Physical Therapy licensure in state of practice.
• Current CPR certification.
• Current driver's license and vehicle insurance or access to public transportation.

Responsibilities

• Evaluates the patient's functional status and physical therapy needs.
• Assists in development of the therapy plan of care.
• Provides services within the scope of practice and in coordination with the health care team.
• Observes and reports patient responses to treatment and changes in condition.
• Treats patients to relieve pain, restore function and maintain maximum functioning.
• Instructs patients and families regarding plan of care, limitations, and progress toward goals.
• Visits patients according to the Plan of Treatment and submits progress notes per policy.
