Job summary

Benefit Home Health Care in Colorado is seeking a licensed Physical Therapist for full-time work in home health. The PT will conduct interdisciplinary assessments, develop and manage individualized care plans, educate patients and families, and coordinate with physicians and care teams to provide services per plan of care.

The role requires Colorado licensure, CPR, and at least two years of PT experience, preferably in home health, with reliable transportation for patient visits.

Qualifications

• Licensed to practice as a physical therapist in Colorado.
• Two years of physical therapy experience; home health preferred.
• Good verbal and written communication and organizational skills.
• CPR certification current.
• Ability to drive and have transportation for agency travel; valid driver’s license and auto insurance.

Responsibilities

• Assumes responsibility for ongoing interdisciplinary assessment and development of the individualized plan of care in partnership with the patient, representative, and caregiver(s).
• Provides patient, caregiver, and family counseling and education.
• Prepares clinical notes and progress summaries based on goals.
• Communicates with the physician and other health care practitioners regarding the home health plan of care.
• Provides physical therapy services according to the physician's plan of care and supervises assistants as needed.
• Participates in discharge planning and quality improvement programs.
