Job summary

UnitedHealth Group is seeking a Physical Therapist for Home Health in Ohio to assess patient needs, develop and implement treatment plans, and monitor progress. You will coordinate with physicians and nurses to deliver care that improves functional abilities and mobility.

Responsibilities include evaluating functional status, treating to relieve pain, and documenting progress while communicating with families and the health care team. A licensed PT and reliable transportation are required.

Qualifications

• Current Physical Therapy licensure in state of practice.
• Current driver's license and vehicle insurance, access to a dependable vehicle, or public transportation.
• Current CPR certification or ability to complete within 90 days of hire.

Responsibilities

• Evaluates the patient's functional status and physical therapy needs; assists in development of the plan of care.
• Provides services within the scope of practice and in coordination with the health care team.
• Observes and reports patient response to treatment and changes in condition.
