Job summary

Ohio's Choice Home Health is seeking a Physical Therapist to perform assessments, determine treatment plans, and deliver interventions to improve patient well‑being in home care settings. You will evaluate progress and adapt care under state regulations, coordinating with the care team.

Responsibilities include documenting visits, communicating with patients and families, and ensuring adherence to the plan of care.

Qualifications

• Current Physical Therapy licensure in state of practice.
• Current driver's license and vehicle insurance or access to reliable transportation.
• Preferred CPR certification or ability to obtain within 90 days of hire.

Responsibilities

• Evaluates patient functional status and needs; assists in therapy plan of care.
• Provides PT services within scope of practice and plan of care.
• Observes and reports patient response to treatment to nurse/physician.
• Treats patients to relieve pain and restore function; maintain progress.
• Educates patient and family about plan of care and progress.
• Completes visits, progress notes, and PT plan of care per guidelines.
• Completes all evaluations and reports outcomes and frequency of care.
• Function in diverse home settings; respect all patients regardless of background.
• Contributes to overall patient wellbeing and therapy goals.
