Qualifications

• Must hold OT license registered in WA.
• Home Health experience preferred but not required.
• Maintain confidentiality with protected information.
• Demonstrate strong time management and problem-solving.
• Proficient with standard office equipment and software.

Responsibilities

• Conduct comprehensive initial and ongoing assessments of patients' physical, functional, cognitive, psychological, and home safety needs.
• Establish measurable, patient-centered goals and develop treatment plans.
• Collaborate with patients, families, caregivers, physicians, and other professionals to plan care.
• Plan, implement, and monitor occupational therapy interventions for an assigned caseload.
• Complete timely clinical documentation including assessments and progress notes.
• Supervise Home Health Aides and other support staff as appropriate.
• Monitor progress and adjust plans to maximize independence and safety.
• Coordinate discharge planning for transitions and ongoing care needs.
• Educate patients and caregivers on adaptive techniques and safety at home.
• Recommend environmental modifications and assistive devices to enhance function.
