Responsibilities

• Providing initial and ongoing assessment related to physical and functional status, home safety, as well as cognitive, psychological and perceptual function to determine baseline and appropriate, achievable goals.
• Developing and revising the plan of care in collaboration with patient/family/caregiver and other care-team members.
• Planning, evaluation, and treatment implementation of assigned caseload.
• Preparing clinical progress notes.
• Supervising and guiding home health aides as appropriate.
• Evaluating outcomes and planning discharge.
• Educating patients and/or families about proper safe use and recommending methods for installation of adaptive equipment.

Territory

North King County (Ballard, Fremont, UW, Greenlake and up through Richmond Beach)

Qualifications

• Must have OT license registered in the State of Washington.
• Current driver's license and car insurance.
• Home Health experience highly preferred but not required.
• Able to maintain strict confidentiality with regard to protected and sensitive information.
• Must be able to solve problems independently and confidently by applying analytical and logical thinking.
• Prioritize workload to meet productivity standards and applicable deadlines.
• Demonstrate flexible and efficient time management techniques.
• Able to use standard office equipment and software proficiently.
