Responsibilities
- Providing initial and ongoing assessment related to physical and functional status, home safety, as well as cognitive, psychological and perceptual function to determine baseline and appropriate, achievable goals.
- Developing and revising the plan of care in collaboration with patient/family/caregiver and other care-team members.
- Planning, evaluation, and treatment implementation of assigned caseload.
- Preparing clinical progress notes.
- Supervising and guiding home health aides as appropriate.
- Evaluating outcomes and planning discharge.
- Educating patients and/or families about proper safe use and recommending methods for installation of adaptive equipment.
Territory
North King County (Ballard, Fremont, UW, Greenlake and up through Richmond Beach)
Qualifications
- Must have OT license registered in the State of Washington.
- Current driver's license and car insurance.
- Home Health experience highly preferred but not required.
- Able to maintain strict confidentiality with regard to protected and sensitive information.
- Must be able to solve problems independently and confidently by applying analytical and logical thinking.
- Prioritize workload to meet productivity standards and applicable deadlines.
- Demonstrate flexible and efficient time management techniques.
- Able to use standard office equipment and software proficiently.
- Able to effectively communicate with individuals from diverse backgrounds and varying educational levels.
