Qualifications

• Current driver's license and car insurance.
• Home Health experience highly preferred but not required.
• Able to maintain strict confidentiality with regard to protected and sensitive information.
• Demonstrate flexible and efficient time management techniques.

Responsibilities

• Providing initial and ongoing assessment related to physical and functional status.
• Developing and revising the plan of care in collaboration with patient/family.
• Supervising and guiding home health aides as appropriate.
