Job summary

Trust Home Care, Inc in Bowie, MD seeks an Occupational Therapist to provide skilled therapy in the home setting. You will assess patient abilities, develop and monitor treatment plans, and coordinate with physicians and case managers to ensure compliance with regulatory requirements.

You will conduct re-evaluations, prepare discharge summaries, maintain accurate records, and educate patients and family/caregivers on goals and procedures.

Qualifications

• Assess patient abilities and limitations thoroughly.
• Provide therapy within physician-approved plan and in compliance with Medicaid and insurer requirements.
• Observe, record and report patient responses to treatment to Case Manager.

Responsibilities

• Perform patient re-evaluation and discharge summary as applicable.
• Maintain records and reports per agency policy; protect patient information as agency property.
• Instruct other health team personnel and family/caregivers in home health procedures.
• Instruct patient/family on goals, procedures, benefits and risks of services; encourage participation.
• Be involved in discharge planning and prepare patient for discharge.
