Job summary

Self-Help for the Elderly is seeking an Occupational Therapist to provide skilled OT services for home health and hospice patients in San Francisco, CA. You will assess function, educate families, and collaborate with an interdisciplinary team to develop and implement individual care plans.

The role requires California OT licensure, CPR, and ability to travel within the service area. Strong communication in English and Cantonese is preferred; Mandarin is a plus.

Qualifications

• Licensed by the California Board of Occupational Therapy.
• Experience in home health, public health, or acute care preferred.
• Ability to work both independently and as part of an interdisciplinary team.
• Bilingual in English and Cantonese; Mandarin is a plus.
• Current CPR Certification and TB clearance; background check required.

Responsibilities

• Provides occupational therapy services to maximize independence in activities of daily living.
• Collaborates with RN, PT, ST, MSW and PCP to coordinate patient care.
• Assesses functional status and safety, conducts home environment evaluations.
• Educates patients and families on safety, ADLs, energy conservation and transfers.
• Documents evaluations, plans of care and progress notes within required timeframes.
• Reports patient responses and changes to the care team and supervisor.
