Job summary

Enhabit Home Health & Hospice is seeking an Occupational Therapist to provide skilled OT in the home setting. The OT will help patients regain daily living skills and basic motor functions under physician orders and the plan of care.

You will collaborate with the health care team, educate family, and deliver services with attention to quality and compliance. A state OT license and graduation from an approved program are required; Medicare home health experience is preferred.

Qualifications

• Must be licensed as an Occupational Therapist/OT in the state.
• Minimum one year of home health experience preferred; can be lesser with approvals based on prior experience.
• Medicare home health experience is preferred.

Responsibilities

• Help patients in the home improve daily living and motor functions.
• Deliver skilled OT services per physician orders and plan of care.
• Educate family and health care team members.
