Job summary

Crossroads Home Health and Hospice seeks an experienced Occupational Therapist to support a growing patient population in San Francisco. You will evaluate patients, administer occupational therapy plans, and help develop care plans with strong communication and time-management skills.

The ideal candidate holds an OT license and has a compassionate, driven approach. Home health experience is preferred; a valid driver’s license may be required.

Qualifications

• Active OT license in good standing.
• Graduate of an OT curriculum accredited by relevant associations.
• Background check clearance required.
• Home health experience preferred but not mandatory.

Responsibilities

• Provide therapy services per treatment plan and physician orders.
• Evaluate treatment effects and patient responses.
• Update plans of care as orders change.
• Develop and adjust treatment plans for each patient.
