Job summary

Atrium is seeking a licensed Occupational Therapist to provide patient-focused care in a home health setting. You will assess, plan, implement, and evaluate care plans, coordinate with a multidisciplinary team, and support patients across ages to improve daily living activities.

Two years of OT experience in acute or rehab settings, active state license, and CPR certification are required. Travel within the local area and a flexible schedule are expected.

Qualifications

• Licensed Occupational Therapist (state license)
• Two years experience as an OT in acute care or rehabilitation
• Current CPR certification
• Driver's license and reliable transportation

Responsibilities

• Perform clinical assessment and develop Plan of Care (POC)
• Document progress notes and care documentation
• Assess ROM, strength, coordination, adaptive equipment, and safety for daily living activities
• Fabricate and maintain adaptive equipment and splints
• Evaluate occupational performance across self-care, work, leisure, and rest
• Collaborate with interdisciplinary team
• Communicate patient information and maintain confidentiality
