Job summary

Atrium Health seeks a Physical Therapist to deliver comprehensive in-home therapy services, manage a patient caseload, and implement individualized treatment plans in collaboration with physicians and the care team.

You will document progress, educate patients and families, and coordinate with the multidisciplinary team to adjust care based on patient needs and goals. Travel to patient homes is required, with appropriate PPE as needed.

Responsibilities

• Manages daily patient caseload in home health and documents care per plan and physician orders.
• Implements therapy modalities, exercise programs, and patient education as directed by the OT/PT.
• Communicates with multidisciplinary team and updates schedules, patient data, and orders.
