Job summary

Atrium is seeking a licensed Occupational Therapist for a temporary contractor role assigned to CenterWell in a home care setting. You will plan, implement, and evaluate individualized care plans to help patients develop or maintain daily living activities, collaborating with an interdisciplinary team.

The role requires current OT licensure, CPR certification, a valid driver’s license, and the ability to travel locally. OTs with 2+ years in acute or rehab settings are preferred.

Qualifications

• Active state license as an Occupational Therapist (OT).
• Proof of current CPR certification.
• Current driver’s license and reliable transportation.
• Minimum 2 years experience as an OT in acute care or rehabilitation.

Responsibilities

• Perform clinical assessments and determine patient needs and care plans.
• Document and revise Plan of Care (POC) and progress notes.
• Assess ROM, strength, coordination, and ADL performance; adapt environmental modifications.
• Fabricate and maintain adaptive equipment and hand splints.
• Evaluate occupational performance in self-care, work, leisure, and rest.
• Coordinate with physicians and care team; protect patient confidentiality.
• Provide education and support to patients and family; promote compliance with payers.
• Perform equipment setup and basic maintenance of OT devices.
