Job summary

Peace and Harmony Homecare LLC is seeking an Occupational Therapist to deliver in-home OT services within a coordinated health care team. The role involves intake processing, home safety evaluation, and development of care plans approved by the physician.

The OT will review patient histories, perform ADL assessments, and collaborate with RN case managers as needed. A MA license, CPR, and background check are required, with preference for homecare experience.

Qualifications

• Must be licensed in MA.
• Minimum of 1 year OT experience, preferably in homecare.
• Criminal background check & CPR certification required.
• If educated outside the US, must meet equivalent OT education and licensing requirements.

Responsibilities

• Receives intake referral information and prepares OT paperwork/tools for visits.
• Maintains ongoing OASIS data sets.
• Obtain medical history from patient or family.
• Perform home safety checks and environmental assessments.
• Conduct OT evaluations or skilled visits and document results.
• Develop and submit Plan of Care (POC) to physician for approval.
• Evaluate ADL and functional abilities; assist physician in assessing function.
• Reassess patient status and update the care plan every 30 days.
• Supervise and coordinate with Home Health Aide as ordered.
• Communicate with physicians and families about treatment modalities and progression.
• Submit accurate paperwork weekly and manage authorization requests.
