Job summary

Peace and Harmony Homecare LLC seeks a licensed Occupational Therapist to deliver in-home OT services. You will collaborate with the health care team, assess patient homes, and educate patients and families to improve independence.

Role requires MA licensure, ACOTE-accredited OT education, CPR, and at least 1 year of homecare experience. Travel to patient residences is expected, with strong documentation and communication skills essential.

Qualifications

• Must have OT degree from ACOTE-accredited program.
• Active MA OT license or eligibility to practice in MA.
• CPR certification and criminal background check are required.
• Minimum 1 year OT experience, preferably in homecare.

Responsibilities

• Receive intake referrals and prepare OT paperwork/tools for visits.
• Perform home safety assessments and patient evaluations.
• Develop and implement Plan of Care (POC) and document progress.
• Collaborate with RN case manager and physicians for care coordination.
• Educate patients and families to improve independence in daily activities.
• Review and update documentation and re-authorization requests as needed.
• Communicate treatment plans to patients and families clearly.
• Travel to patient residences for in-home visits.
• Maintain licensure and pursue ongoing professional development.
