Job summary

Peace and Harmony Homecare LLC seeks an Occupational Therapist to provide in-home services under supervision with RN case management. You will perform OT assessments, develop and implement care plans, and collaborate with physicians to optimize patient outcomes.

Applicants must hold a MA OT license and have at least 1 year of OT experience, preferably in homecare, with CPR and background checks current. Travel to patient homes is required.

Qualifications

• ACOTE-accredited OT program graduate eligible to take NBOT exam.
• Massachusetts OT license current.
• Minimum 1 year experience as an OT, preferably homecare.
• Criminal background check and CPR certification.
• Foreign-trained graduates must meet US-equivalency and licensure timing requirements.

Responsibilities

• Receives intake referral information and prepares OT paperwork/tools for visits.
• Performs home safety checks and environmental assessments.
• Conducts OT evaluations and skilled visits with documentation.
• Develops and submits Plan of Care to physician for approval and implementation.
• Evaluates ADL performance and assists physician in function levels.
• Revises and signs care plans during certification periods and when nursing discharges.
• Supervises Home Health Aide per state/federal requirements if OT is sole discipline.
