Job summary

Peace and Harmony Homecare LLC seeks a licensed Occupational Therapist to deliver in-home services under supervision of the RN case manager and Administrator. The role includes conducting home assessments, evaluating function, developing a Plan of Care, and coordinating with physicians to ensure timely, patient-centered care.

Requires MA licensure and active CPR certification. Ideal candidates have at least one year of OT experience, preferably in homecare, and must pass a background check.

Qualifications

• Must be licensed to practice OT in Massachusetts.
• Minimum 1 year of OT experience, preferably homecare.
• Criminal background check and current CPR certification.

Responsibilities

• Receive intake referral information and prepare OT paperwork/tools for visits.
• Conduct home safety checks and environmental assessments.
• Perform OT evaluations and skilled visits, document findings.
• Develop and submit Plan of Care to physician for approval.
• Review ADLs and assist physicians in evaluating function.
• Reassess patient status and adjust care plans every 30 days.
• Supervise Home Health Aides as required and document supervision.
• Communicate with patient, family, and other disciplines about treatment progression.
• Submit weekly paperwork and manage re-authorization requests as needed.
• Maintain licensure and pursue ongoing education to enhance home care OT services.
