Job summary

BAYADA Home Health Care is seeking a Medical Social Worker (MSW) for a per diem opportunity to perform home health visits from our Fayetteville office. Responsibilities include home visits in Cumberland, Harnett, and Hoke counties and collaborating with a multidisciplinary team to plan care.

Qualifications include a Master of Social Work with at least 1 year in health care; CSWE accreditation; English proficiency; and ability to travel.

Qualifications

• Must hold a Master of Social Work (MSW).
• At least 1 year of social work experience in a health care setting.
• CSWE accreditation required (or demonstrated).
• Home care experience a plus, not required.
• Able to read, write and communicate in English.

Responsibilities

• Make home visits to clients in designated geographic territories.
• Assess social and emotional factors related to clients’ health problems.
• Provide clinical social evaluations and plan interventions.
• Identify community resource referrals for patient and family needs.
• Advocate for client and family through the community system.
• Participate in development of total plan of care with multidisciplinary team.
• Develop and maintain contracts with public/private agencies as resources.
• Educate patients and families about community resources to promote health and independence.
• Accurately document observations and interventions using an EMR tablet.
