Ideal candidates hold an MSW with CSWE accreditation and at least 2 years of community-based experience; strong English communication and ability to travel are required. Prior home health EMR familiarity is a plus, and travel is expected as assigned.

Qualifications

• Master's in Social Work required; CSWE accreditation preferred.
• 2+ years of community-based experience.
• Strong written and spoken English essential.

Responsibilities

• Make home visits to clients in designated geographic territories.
