Job summary

BAYADA Home Health Care is seeking a per diem Medical Social Worker (MSW) for home health visits in Worcester, Auburn, Charlton, Uxbridge and surrounding towns. You will work with a multi-disciplinary team to help clients receive essential care at home.

Requirements include a Master’s Degree in Social Work and current MA LCSW/LICSW licensure. Prior home care experience is preferred but not required. Base pay ranges from $63.00 to $68.00 per hour, depending on qualifications.

Qualifications

• Master’s degree in Social Work required.
• Massachusetts Licensed Social Worker (LCSW/LICSW) required.

Responsibilities

• Make home visits to clients in designated geographic territories.
• Provide clinical social evaluations and plan interventions based on findings.
• Identify community resource referrals to address practical and environmental needs.
• Advocate for clients and families to navigate the health system.
• Participate in total plan of care development with BAYADA team.
• Develop and maintain contracts with public and private agencies as resources.
• Educate patients and families on community resources to support health and independence.
• Document observations and evaluations using a tablet.
