Medical Social Worker (MSW) – Home Health

Company: BAYADA Home Health Care

Location: Roxboro, NC

Job Type: Full-time (per diem opportunity on a per visit basis)

We are seeking an energetic and experienced Licensed Medical Social Worker, MSW, to perform home health visits in designated territories. The role is integral to a multi-disciplinary team providing skilled nursing and rehabilitative care.

Responsibilities

• Make home visits to clients in designated geographic territories.
• Assist the team with understanding social and emotional factors related to clients' health problems.
• Provide clinical social evaluations and plan appropriate interventions based on evaluation findings.
• Identify community resource referrals to address practical and environmental needs.
• Act as advocate for clients and families navigating the community system.
• Participate in the development of the total plan of care with the multidisciplinary team.
• Develop and maintain contracts with public and private agencies.
• Educate and instruct patients, families, or other representatives about community resources.
• Accurately document observations, interventions, and evaluations using a touch‑pad tablet.

Qualifications

• Licensed by the STATE Board of Social Work Examiners at the Master (LMSW) or Independent (LISW) level.
• Minimum of a Master’s in Social Work with at least 2 years of community-based experience.
• Accreditation by the Council on Social Work Education (transcript or degree).
• Home care experience a plus.
• Excellent written and verbal communication in English.
• Ability to work independently and manage time effectively.
• Strong interpersonal skills.
• Solid computer skills; prior experience with electronic medical records preferred.
• Ability to travel to cases as assigned.
