Job summary

BAYADA Home Health Care in Bucks County, PA is seeking a licensed Medical Social Worker (MSW) for a per diem home health visits role. The MSW works with a multidisciplinary team to support patients living at home.

Responsibilities include visiting clients in designated territories, assessing social needs, coordinating community resources, documenting care, and contributing to the total plan of care with BAYADA clinicians.

Qualifications

• Must hold PA LMSW or LISW license and MSW degree.
• Experience with home care is a plus but not required.
• Strong English communication and ability to work independently.

Responsibilities

• Make home visits to clients in designated geographic territories.
• Assess social and emotional factors related to clients' health problems.
• Provide clinical social evaluations and plan interventions.
• Identify community resource referrals for patient and family needs.
• Advocate for clients and navigate them through the community system.
• Collaborate with BAYADA clinicians to develop the total plan of care.
• Develop and maintain contracts with agencies for patient resources.
• Educate patients and families about community resources and health maintenance.
• Document observations and interventions using EMR on a tablet.
