Job summary

REM Community Services, a part of the Sevita family, seeks a licensed practical nurse to provide prescribed medical treatment and personal care under the direction of an RN or physician.

This full-time role offers a $23.00 per hour pay, with a day shift from Monday through Friday, 8am–5pm. On-call for weekends and evenings as needed. A valid driver’s license and a clean motor vehicle record are required.

Qualifications

• Associate degree in nursing or state nursing certificate.
• At least 1 year experience working with the population served in a nursing capacity.
• Current LPN/LVN licensure for state of residence; CPR/FA and HIV/AIDS certification, BBP, ACT training.

Responsibilities

• Provides basic care and medical treatment to individuals.
• Completes health assessments and inputs health diagnosis to RN/physician for care plan.
• Monitors and records medications; liaises with pharmacy; administers medications as required.
• Oversees staff who administer medication; conducts quality assurance on records.
• Provides emergency coverage and/or care as needed.
• Maintains confidentiality and follows universal precautions and policies.
• Works with external agencies to coordinate services and ensure consistent treatment.
• Provides training for staff on medication administration and infection control.
• May assist with household activities and transport to activities and appointments.
• Performs other related duties as required.
