Mount Sinai Health System seeks a Palliative Care RN to deliver home-based symptom assessment and treatment for patients with advanced illness. This part-time day-shift role operates in New York, NY, collaborating with an interdisciplinary team to provide high-quality, evidence-based care in the patient’s home.

Applicants should have a BSN (or ADN with BSN progression), 2+ years of relevant experience, an active NY RN license, BCLS certification, and CPR/ACLS where possible.

Qualifications

• BSN required; ADN acceptable if enrolled in BSN program.
• 2+ years of experience managing patients with medically complex and chronic/severe illness.
• Experience working in interdisciplinary care teams preferred.
• Current RN License in New York State.
• Basic Life Support (BCLS) certification required.
• CPR/ACLS certification preferred.

Responsibilities

• Work as a member of an interdisciplinary team assessing complex care needs of patients and families.
• Visit patients in their homes to complete guided assessments and support medical management.
• Provide supportive individual and family counseling to reduce anxiety and symptoms.
• Engage patients and caregivers about goals of care and develop treatment plans.
• Monitor and coordinate care plans with the clinical care team.
• Evaluate medical needs for a caseload of patients and provide direct in-home care.
• Facilitate remote visits by providers as needed.
• Communicate the plan of care to consulting clinicians and interdisciplinary teams.
• Educate patients and families on the trajectory of illness and medication use.
• Participate in weekly interdisciplinary team meetings and document as required.

Education

Bachelor’s Degree in Nursing (BSN) required

Associate’s Degree in Nursing (ADN) acceptable if matriculated in a BSN program
