Intermountain Health Utah Valley Hospital is seeking a part-time Social Work Care Manager I to perform psychosocial assessments, develop care plans with the care team, and provide crisis counseling as needed.

This role supports patients and families, coordinates resources, and may require driving for community visits. A $500 sign-on bonus is offered to external applicants with 1+ year of experience.

Qualifikationen

• MSW degree from an accredited institution (verification required).
• Current state social work licensure (CSW/LCSW) or equivalent prior to supervision hours completion.
• Basic computer proficiency with word processing and spreadsheet software.
• Strong written and verbal communication skills.
• Demonstrated critical thinking ability.
• Caregivers who perform home/community visits must maintain BLS, have a valid driver’s license, auto insurance, and reliable transportation.

Aufgaben

• Performs psychosocial assessments and develops care plans with the care team.
• Provides crisis counseling and brief therapeutic interventions as needed.
• Delivers individual, group, and family therapy and conducts psychosocial assessments.
• Identifies mental, emotional, and behavioral disorders and connects patients to services.
• Educates and advocates for patients and families to facilitate safe transitions of care.
