Qualifications

• Bachelor's Degree in Social Work, Human Services, Nursing, or related field preferred (non-licensed candidates accepted).
• Prior healthcare experience in case management or social services preferred.
• Knowledge of Medicare, Medi-Cal, and Medicaid programs and benefits.
• Familiarity with electronic medical record systems like PCC preferred.

Responsibilities

• Communicate with residents and families about plans of care and treatment protocols.
• Coordinate admissions, discharges, and referrals to optimize patient flow.
• Perform utilization review to ensure cost-effective care.
• Document accurately and maintain charting to support care plans.
• Collaborate with physicians, nurses, payers, and families to promote coordinated care.
