Acenda Integrated Health is seeking a Care Manager on an hourly basis to work with clients, communities, and providers to ensure effective care plans and timely service connections. The role emphasizes connecting clients to housing, employment, mental health, and primary care resources, with on-call crisis support as needed.

The position supports integrated case management services across hospital to community transitions, with documentation timely and adherence to program goals.

Qualifications

• Master’s Degree in social work or related field required.
• Valid driver’s license with acceptable driving record.
• Ability to analyze work, set goals, develop plans, and manage time effectively.
• Relevant experience in case management or related behavioral health services.

Responsibilities

• Provide face-to-face targeted case management and community integration.
• Assess, plan, link services, monitor progress, and advocate.
• Maintain documentation within state contracts and program manual.
• Coordinate hospital discharge planning and post-discharge follow-up.
