Job summary

Sevita's Pennsylvania Mentor program is seeking a Case Manager Foster Care to coordinate and oversee services for youth in residential and treatment foster care settings. You will manage case plans, monitor care quality, and collaborate with families and external providers to support individualized plans.

The role requires a BS/BA in social work, criminal justice, or education and at least 2 years of case management with at-risk youth.

Qualifications

• Bachelor's degree in social work, criminal justice, or education required.
• At least 2 years' case management experience, including at-risk youth.
• Current driver's license, car registration, and auto insurance.
• Licensure and training as required by state (CPR, First Aid, Behavioral Intervention Techniques, etc.).
• Reliable, compassionate attitude and commitment to quality.

Responsibilities

• Coordinate development, review and implementation of services and case management.
• Oversee services to individuals and families; monitor quality of care and regulatory compliance.
• Assist in developing quality services and engaging activities to meet individual needs.
