Job summary

21st Century Home Health Services in California seeks a Health Field Physical Therapist to build core clinical skills in a home health setting. You will focus on follow-up visits under supervision of a PT Case Manager, gaining insight into company operations.

You are expected to provide hands-on therapeutic interventions, perform home evaluations, collaborate with the interdisciplinary team, and participate in weekly case conferences.

Qualifications

• Current, unrestricted California Physical Therapist license.
• Current BLS/CPR certification.
• Valid CA driver’s license with a clean driving record.
• Graduate of an accredited Physical Therapy program (DPT preferred).
• One year of experience as a Physical Therapist preferred; new graduates may be considered.
• Home health experience is highly desirable.
• Ability to work well within an interdisciplinary team.

Responsibilities

• Provide hands-on therapeutic interventions in home settings (gait training, balance, neuromuscular re-education, therapeutic exercises).
• Conduct detailed patient evaluations in the home environment.
• Implement plans of care under PT Case Manager supervision.
• Document care within 24–48 hours per care plan and standards.
• Educate patients and caregivers on mobility, safety, and equipment use.
• Collaborate with the PT Case Manager and interdisciplinary team to coordinate care.
• Meet productivity and ongoing professional development requirements.
• Participate in initial evaluations, reassessments, and discharge planning.
