21st Century Home Health Services Inc. in Dixon, CA is seeking an entry-level Health Field Physical Therapist to develop core clinical skills in home visits under a PT Case Manager. The role focuses on follow-up visits with coaching and mentorship from leadership.

You will build foundational skills, work within an interdisciplinary team, and gain exposure to operations and compliance in a growing home health organization.

Qualifications

• Current, unrestricted California Physical Therapist license.
• BLS/CPR certification.
• Valid California Driver's License.
• Graduate of an accredited Physical Therapy program.
• One year of PT experience preferred; new graduates considered.
• Home health experience is highly desirable.
• Comfortable with electronic documentation and team care.

Responsibilities

• Provide hands-on PT interventions in home health settings, including gait and balance training.
• Implement individualized plans of care under a supervising PT Case Manager.
• Document patient care accurately within 24–48 hours per plan of care.
• Educate patients and families on mobility, safety and use of adaptive equipment.
• Collaborate with RN, OT, ST, MSW and HHA to coordinate care.
• Participate in evaluations, discharge planning, and interdisciplinary conferences.
• Maintain privacy and comply with health data standards; meet productivity targets.
