Job summary

21st Century Home Health Services (21HHS) seeks a Health Field Physical Therapist to visit patients at home across the Bay Area. You will provide gait training, balance retraining, and therapeutic exercises under a PT Case Manager while documenting care within 24–48 hours.

The role requires a California PT license, BLS/CPR, and a valid CA driver’s license. Home health experience is preferred, with strong teamwork and communication essential for patient-centered care.

Qualifications

• California PT license required and current CPR/BLS certification.
• Graduate of an accredited Physical Therapy program with readiness for home health practice.
• Experience in home health is highly desirable and beneficial.
• Strong communication and documentation skills essential for patient care plans.
• Ability to work within an interdisciplinary team (RN, OT, ST, MSW, HHA).

Responsibilities

• Provide hands-on interventions including gait training, balance and neuromuscular re-education in a home setting.
• Assist with prosthetics/orthotics, wheelchairs, and home safety equipment management.
• Perform detailed home-based patient evaluations and ongoing assessments.
• Develop and implement plans of care under supervision of a PT Case Manager.
• Document care accurately within 24–48 hours in compliance standards.
• Educate patients and families on mobility, exercises, safety, and adaptive devices.
• Collaborate with the PT Case Manager to update care plans and treatments.
• Participate in interdisciplinary coordination and notify relevant team members.
• Use equipment efficiently while maintaining cost awareness and quality care.
• Engage in ongoing professional development and clinical trainings.
• Take part in initial evaluations, reassessments, and discharge planning with the Case Manager.
• Participate in on-call or weekend coverage as required.
• Maintain patient privacy and dignity in all documentation and care activities.
