21st Century Home Health Services is seeking an entry-level Field Physical Therapist to visit patients in their homes and build core clinical skills under supervision. The role emphasizes follow-up visits, training, and mentorship while supporting a growing, high-quality home health program.

Anticipated coverage area includes Watsonville, with supervision from a Physical Therapist Case Manager and strong emphasis on documentation, compliance, and patient safety.

Qualifications

• Current, unrestricted California Physical Therapist (PT) License.
• Current BLS/CPR Certification.
• Valid CA Driver’s License with a clean driving record.
• Graduate of an accredited Physical Therapy program.

Responsibilities

• Provide hands-on therapeutic interventions in home health settings.
• Manage patients with assistive devices and safety equipment.
• Conduct detailed evaluations and assessments in patients’ homes.
• Implement plans of care under supervision of a PT Case Manager.
• Document patient care accurately within 24–48 hours.
• Collaborate with the interdisciplinary team to coordinate services.
